In compliance with the Family Educational Rights and Privacy Act (FERPA), these records are maintained with the highest standards of confidentiality and security.

Your information is protected and will not be accessed, disclosed, or shared without your explicit written consent except in cases where disclosure is legally mandated or expressly authorized by FERPA regulations.

You have the right to review your educational records, request corrections if you believe the information is inaccurate, and be informed of who has accessed your data. The college is committed to ensuring you understand your privacy rights and can exercise them fully. 

Academic Notice

We care about your success at 91果冻制片厂.  Sometimes students encounter challenges, like Academic Notice, on their educational journey.

What is an Academic Notice?

Being placed on academic notice is a notification from the College that we are concerned about your academic performance and progress. Although you may have experienced some academic challenges, each semester offers an opportunity for progress and improvement.

What are the minimum academic and progress standards?

  • Academic standard:  Students are required to maintain an overall grade point average of 2.00.
  • Progress standard:  Students are required to complete at least fifty percent of overall credits attempted. 

How are academic and progress standards determined?

  • Standards are determined using institutional units on the official academic record at West Valley-91果冻制片厂 Community College District. 

How does credit for prior college coursework impact my academic and progress status? 

  • Coursework completed at previously attended institutions that has undergone a transcript evaluation is reflected on the official transcript at WVMCCD for 'Transfer Credit Accepted by the Institution' section to include:
    • Name of the college or university
    • Term of transfer credit
    • Earned credit hours
    • GPA hours
    • Quality points
    • Transfer GPA 
  • Transcript evaluations of at least 12 units will initiate a review of the academic and progress standards for institutional credit-level courses.  

When are academic and progress standards determined?

  • Academic and progress standings are calculated at the end of each fall term and spring term for students who attempt a minimum of twelve cumulative units. 
  • The college notifies students via email of their academic/progress standings.
  • Academic notice, progress notice, subject to dismissal and dismissal statuses appear on the student's semester grade report and the transcript of academic work.
Academic/Progress Notice 1


A student is placed on Academic/Progress Notice 1 if their academic performance at 91果冻制片厂 and/or West Valley College(s) meets the following:

  1. A student has attempted a total of 12 or more semester units, at 91果冻制片厂 and/or West Valley College(s), AND;
  2. The student鈥檚 cumulative grade point average is below 2.00. OR;
  3. The student鈥檚 overall course completion rate is 50% or less of the units attempted.
  4. The one in 鈥淎cademic Notice鈥 simply means it鈥檚 a first term.

Academic Progress Notice 2

A student is placed on Academic/Progress Notice 2 if their academic performance at 91果冻制片厂 and/or West Valley College(s) meets the following:

  1. A student has attempted a total of 12 or more semester units, at 91果冻制片厂 and/or West Valley College(s), AND;
  2. The student鈥檚 cumulative grade point average is below 2.00. OR;
  3. The student鈥檚 overall course completion rate is 50% or less of the units attempted.
  4. The two in 鈥淎cademic Notice鈥 means it鈥檚 a second term with GPA below 2.00 or overall completion of 50% or less.

Subject to Dismissal

A student shall be placed on Subject to Dismissal status if their academic record of performance at 91果冻制片厂 and/or West Valley College(s) meets the following criteria:
1. A student has accumulated a total of 12 or more semester grade units, at 91果冻制片厂 and/or West Valley College(s), and
2. The student鈥檚 cumulative grade point average is below 2.00 for a second consecutive semester. OR
3. The student鈥檚 overall course completion rate is 50% or less of the units attempted for a second consecutive semester.

Dismissal

Academically disqualifed students are dismissed from the college.  A student shall be placed on Dismissal status if their academic record of performance at 91果冻制片厂 and/or West Valley College(s) meets the following criteria:
1. A student has accumulated a total of 12 or more semester grade units, at 91果冻制片厂 and/or West Valley College(s), and
2. The student鈥檚 cumulative grade point average is below 2.00 for a third consecutive semester. OR
3. The student鈥檚 overall course completion rate is 50% or less of the units attempted for a third consecutive semester.

Readmission/Reinstatement

Academically disqualifed students on dismissal status are eligible for reinstatement when:

1. The student takes a leave of absence for the fall or spring term following dismissal status.  This leave of absence during a primary term (fall or spring) qualifies the student for reinstatement and a return to West Valley-91果冻制片厂 Community College District.  Reinstated students will be re-assessed for academic (GPA) and progress (completion) requirements upon completion of the reinstatement term and any subsequent terms. 

2. The student has submitted an  to the Student Petitions Committee and the committee has approved the Appeal for Readmission.  Once reinstated, students may return to West Valley-91果冻制片厂 Community College District.  Reinstated students will be re-assessed for academic (GPA) and progress (completion) requirements upon completion of the reinstatement term and any subsequent terms.

Academic Notice and Registration Dates

Students on a form of Academic Notice, Progress Notice, Subject to Dismissal or Dismissal are assigned registration dates as follows:

  • Academic/Progress Notice 2 students: May register during Group 4.
  • Subject to Dismissal students: May register during Group 4.
  • Dismissed students: May register during Group 4, with an approved .

Students with documented extenuating circumstances may submit a Priority Registration Appeal.   

Academic Notice and Financial Aid

  • Financial aid has a different Satisfactory Academic Progress policy.
  • Please check with financial aid to check the status of your grant and loan eligibility.

Release of Student Records

No instructor, official, employee, or governing board member shall authorize access to student records to any person except under the following circumstances:

Student records shall be released pursuant to a student's written consent.

"Directory information" may be released in accordance with Board Policy, unless otherwise specified by the student. Students should complete a "Directory Information Opt-Out Form" and submit it to the Admissions & Records Office to withhold the provision of Directory Information

Directory information shall include:

  • Student name.
  • Student participation in officially recognized activities and sports, including weight, height, and high school of graduation of athletic team members.
  • Degrees and awards received by students, including honors, scholarship awards, athletic awards, and Dean's List recognition.

Who Can Request a Transcript Evaluation?

You can request an evaluation if you鈥檝e completed or are taking at least 12 units at 91果冻制片厂.

How to Request an Evaluation

  1. Send official transcripts from all colleges you鈥檝e attended to:

Transcripts must come directly from the school or in a sealed envelope.

  1. Once received, fill out and submit the Request for Transcript Evaluation form.

How Long Does It Take?

Evaluations take about 2鈥3 weeks. You鈥檒l get an email update when it鈥檚 done.

Important Info

  • Only courses with final grades (A, B, C, D, F, CR, P) count.

  • Incomplete, In Progress, or Report Delayed classes don鈥檛 count yet.

  • Withdrawals, No Credit, or No Pass don鈥檛 count.

  • Your GPA includes all letter grades鈥攏o exceptions.

  • Evaluations become part of your permanent record.

Can You Improve Your GPA?

Yes! Check with past schools about removing low grades. If approved, request a new transcript and resubmit your evaluation.

Foreign Coursework?

Get your transcripts evaluated by an approved service like IERF, NACES, WES, IIC, or IESCAREE.

What Are Catalog Rights?

Catalog rights lock in your degree rules from your start year. Take at least one class each academic year to keep them. If you skip a year, your rights reset to the current catalog when you return.

Do You Need an Evaluation?

If you want to transfer credits from another college, yes. Check with a counselor first to be sure.

About Your District Transcript

The West Valley-91果冻制片厂 District issues one combined transcript showing courses from both 91果冻制片厂 and West Valley College including credit, International Student Services, and Community Education classes. Midterm grades won鈥檛 show. Records before Fall 1976 must be requested directly through West Valley College.

Paper Transcripts

Use the transcript order form to request a hard copy. Payment may apply.

Rush Orders: Available in person for a $20 fee per order. Not processed on weekends or holidays. No refunds.

Costs

  • The first two (2) standard paper transcripts (from 91果冻制片厂 or West Valley) are free.

  • Additional paper transcripts cost $6 each.

  • Electronic/PDF transcripts always cost $6; no free versions.

Processing Time

Orders are processed and mailed or sent electronically within 5鈥10 business days, excluding weekends and holidays.

Transcripts from Other Colleges

91果冻制片厂 cannot forward or release transcripts from other schools. You must request those directly from the original institution.

Important Notes

  • Hold for Grades: Transcripts are delayed until current semester grades are posted.

  • Delivery: Once mailed, delivery is handled by the U.S. Postal Service and may be delayed or lost. You鈥檙e responsible for reordering if lost.

  • Pickup: Paper transcripts can be mailed or picked up at Admissions and Records (SEC 118). They are never faxed or emailed.

True Name

Sometimes a person's given name doesn't reflect who they are. We recognize that you may identify by a name that is different from your legal one. 91果冻制片厂 respects students' names and recognizes that students may be unable to go through a legal name change during their time here. We are committed to using a student's True name (first name) during their time as a 91果冻制片厂 student.



Using Chosen Names

You can change your first name to reflect your True name. It will appear on documents, such as class rosters, and on your Canvas page.

True Name FAQs

Q: What is a True or Preferred Name? 

A: Your True or Preferred Name is the first name an individual at 91果冻制片厂 (employee or student) wishes to be known by in the community. A True/ Preferred name differs from an individuals legal first name and is not meant to be used to record a nickname (e.g. Becky for Rebecca.) Surnames can be changed only through a legal name change. 

Q: What is a Legal Name? 

A: Your Legal Name is the name that appears on an individual's passport, driver's license, state identification (i.d.), birth certificate, or U.S. Social Security Card. 

Q: Who can elect to use a Chosen Name? 

A: Anyone at 91果冻制片厂, employees and students. 

Q: Can I request a new student ID card with my True/ Preferred name? 

A: Yes. Students updating their name to reflect their True/ Preferred first name may have a free one-time update of their student ID card to reflect their True name. 

Q: Can I change my middle name? 

A: Not yet. This service only applies to first names currently. 

Q: Can I delete my True/Preferred name? 

A: Yes, the same way you added your True/Preferred name you can ask to remove it. 

Q: How do I change my legal name? 

A: You will need to do that via the Court system in the State of California. Please visit the California Courts website on鈥.

Q: Where will my True/Preferred Name be used?

A: Your True name will display in multiple areas. This includes the following areas: 

  • ID Card* 
  • Class Rosters

Q: Are there any restrictions?

A: Choice of a preferred first name must be appropriate and cannot be an attempt at misrepresentation or fraud. Preferred names submitted that are considered offensive, obscene, lewd, or disruptive to the learning environment will not be approved.

Q. Are there situation where I cannot use my True/Preferred name?

A. The use of Legal Name is required for certain offices that perform areas which legally necessitate the use of a Legal Name. These include: 

  • Office of Visa and Immigration Services (OVIS)
  • Campus Billing 
  • Financial Aid
  • Official Academic Transcript
  • Athletics
  • Safety and Security
  • Human Resources/ Student Employment Office 

There may be some situations where it may be necessary to clarify that your preferred name is different from your legal name. Examples of this include official interactions with law enforcement, business processes, verification of medical records, academic transcripts, and financial aid. 

Under the鈥Family Educational Rights and Privacy Act (FERPA)鈥a student's name, including one's True Name, may be disclosed to the public as "directory information" unless the student opts not to permit such disclosure by completing a鈥疪equest to Prevent Disclosure of Directory Information. 

Credit by Examination

(Title 5, 55050, Ed Code Section 70901 and 70902, Board Policy 4235, Administrator Procedure 4235)

Credit by examination allows the student to use acquired knowledge, abilities and competencies to challenge certain courses for units. Credit will be granted to any student who meets the requirements for credit by exam and who successfully passes an examination approved and conducted by the proper authorities of the college.

Grading shall be according to the regular grading scale: except Pass/No Pass Option shall be offered, as an option, if it is ordinarily availabe for the course. The regulations governing credit by examination are listed below.

Limitations

Students are cautioned that any credits obtained by any of the methods listed below, while accepted by 91果冻制片厂, may not be acceptable for credit by any other institution, public or private.

It is the student's responsibility to check with other institutions to determine the acceptability of any credit earned by examination.

Those students receiving 70% or greater on the Credit by Examination will receive credit for the course. Courses that allowed for Credit by Exam are listed online.

Regulations for Credit by Examination

  1. The credit by exam status for each course is determined by each department and is noted on the official course outline of record.
  2. The student must be registered and in good standing at 91果冻制片厂.
  3. The student can demonstrate that he/she is especially qualified, through previous training or instruction, to complete such examination.
  4. A maximum of twelve (12) semester units toward an associate degree may be earned by courses for which credit has been earned by examination.
  5. A student may take an examination only once and the grade received on the examination will be recorded on the permanent record of the student. The student's academic record will clearly indicate that the credit was earned by examination.
  6. Grading for the examination shall be according to the regular grading system except that students shall be offered a "pass-no pass" option if that option is ordinarily available for the course.
  7. The units earned through such examination shall not be used to satisfy the minimum twelve (12) semester hours of credit in residence required for an associate degree.
  8. Credit by examination is not applicable to a meeting of such load requirements a Selective Service deferment, Veteran's or Social Security benefits.
  9. Examinations are only given during the Fall and Spring regular semesters and request must be received by the sixth week of the semester to be processed during that semester.

What do I do next?

  1. Fill out the Credit by Examination form (form is online). Once filled out then submit it to Admissions & Records by the sixth week of the Fall or Spring semester.
  2. You will receive confirmation by email from the instructor assigned; a date and time to take the exam. Allow for a 2-hour time period.
  3. A 91果冻制片厂 Student ID is required as proof of identity prior to taking the test. No other identification will be accepted.

You MUST apply following the procedure above. No drop-ins. You will be called to confirm your attendance at the appointed time. 

Fall and Spring Schedule for Credit by Exam:

  • Exams will be scheduled on a case by case basis.
  • Deadlines: Student must file the application, with the Admissions & records department prior to the end of the sixth week in Fall or Spring regular semesters.
